Abe Kunstler is one of two things - either a woman passing as a man or a man who has had some tragedy befall him that means he is unable to reach puberty. This much is clear from the first impression but the answer is not revealed (and quite likely won't be until then end; if at all). Personally I think Abe Kunstler was born female but self-identifies as male and is doing his best to achieve it.
To be honest I didn't really enjoy the opening chapters of this book and they haven't really instilled in me the urge to learn more or read further. The telling is all a little self-conscious and you can feel the author choosing the words with care. The craft of the telling obscures the story and disassociates the reader from the story instead focusing you on the words used and the sense of a scene being created.
